Procedure/ Process/ Rule :-

Processing payment – RM20.00 and cost for application form management - RM5.00 for every application.

You are not allowed to do business as long as your license not approved by Kuala Pilah District Council.

Incomplete application will be rejected.

Complete application need to be sent to Public Health Division and License MDKP. Documents needed :-

image A copy of business registration.
image 6 copies of business location plan.
image A copy of agreement letter from building's owner (if not belong to you).
image A copy of premise floor plan (factory and food premises only)
image A copy of qualification certificate (CF). (New building only)
image A copy of deed
image A copy of identity card1
image Letter from MIDA (if you are international investor).
image 2 pictures in passport size.
image District council can reject the license if its break the procedures, place not appropriate.
